All Bidders must submit Tender through “Tender Booklet” prepared by procuring entity and abide by the terms and conditions stated in the “Tender Booklet”.
The Bidders shall submit with its Tender the following additional documents :
A The bidder must be a Bangladeshi Citizen and must submit attested photocopy of Nationality Certificate / National Identity Card.
B Valid Taxpayer Identification Number (TIN) certificate and Income Tax Clearance Certificate for the Fiscal 2017-18.
C Financial Solvency certificate stating amount from any reputed Bank (The certificate should be issued after 31-01-2018). Certificate Stating “Sound & Solvent” is not acceptable and bids submitting that type of certificate will be rejected.
D Bank Statement (Original Computer Sheet) from 01-07-2017 to 31-01-2018. Bidders bidding for different lots should attach the original copy of the bank statement with one lot and attested photocopy of the same with other lots.
E Bidders must be experienced in supplying / repairing similar items / goods worth stated below to / at Tertiary level Hospital / Specialized Govt. Hospital / Civil Surgeon Office / Any Government Hospital in the last 03 (Three) years. Experience certificate must be submitted.
