| Brief Description of Assignment |

Assess current situation of persons with disability in the context of persons with disabilities in termsof their development compared to SDGs with recommendations way forward for effective planning and inclusion of persons with disabilities to achieve SDGs 2030. The study aims- (1) to explore current health and rehablilitation situation of persons w ith disabilities; (2) to identify existing realities and key barriers within the existing govemment health service delivery system including but not limited to financing, governance, inter-ministerial aoordination, technology, medicine, instruments, infrastructure and workforce; (3) to identify the gaps within national health policy and legislations; (4) to identify gaps in the area of community awareness, participation and mobilization in relation with access to health services; (5) to review advancement of persons with disabilities against Sustainable Development Goals (SDGs) |

The applicant shall have to submit the EOI with the following documents: (1) Category of the Firm: Research/Survey Organization/Consulting Firm; (1) Brief description of the firm with proof working in the relevant areas and of conducting Research/Survey; (2) VAT (3) TIN (4) Trade license (5) Ten Years experience in the relevant survey/research; (6) List of Major clients working with contact reference; (7) Bank Solvency certificate; (8) name of the central key personnel with C.V. who will be involved with the assignment; (9) Relevant documents in favor of organizational strength of the firm. |
