INFORMATION FOR APPLICANT

| Brief Description of Assignment |
: |
a) Agreement with a reputed and established insurance company which will provide our employees for life with ADB, PTD, PPD and hospitalization insurance plan.
b) The above insurance plan will include a life insurance & treatment expenses of hospitalization due to any cause including maternity with registration and admission fees.
c) Emergency hospitalization services.
d) Day care Service: Surgery, different procedure, Dialysis, Chemo Therapy & Radio Therapy, Physiotherapy (as per doctor advice), Labor room, Dressing, Nebulization, 02 (Oxygen) therapy etc.
e) Ambulance Service to and from the hospital within the city/District.
f) There should be no preliminary medical checkup before enrollment
g) Notification of hospitalization can be done within the period of hospital stay or before.
h) Insurance Company will have to pay all hospital bill as per contract.
i) Reimbursement facilities from non designated hospital.
j) Facilities for oversees treatment – Without Setting any standard of expenses reimbursement in Bangladeshi Taka up to maximum limit of the employees which was actually spent in oversees hospital.
k) 15 Days pre and post hospitalization treatment expenses-ie.consultation, medicine and investigation cost.

a) Experience regarding agreement with other govt., autonomous body & non govt. organization with number of client list.
b) 10 years experience as insurance company
c) Contract with minimum 20 renowned hospitals in Dhaka city and 20 renowned hospitals country wide.
d) Satisfactory payment history of insurance claim.
e) Bank solvency certificate regarding ability to pay minimum taka 5 Crore per month for insurance claim.
